IMPORTANT: Requirements (unless specified to be submitted prior to admission) are to be submitted to the Registrar’s Office once online enlistment has been confirmed and student no. has been issued. Enrollment is conditional pending the submission of documents (original hard copies) in-person or via courier. Advisory: Once limited face-to-face classes are implemented, the Commission on Higher Education requires those who shall undergo in-person classes (refer to vaccine eligibility of the Dept. of Health) to be fully vaccinated against COVID-19. The same shall be required for allied medical students who shall undergo internships in medical facilities.
