PROFILE
The Registrar’s Office as part of the Student Support Services Division is the repository of highly important, delicate and confidential documents. The office is responsible for preserving and safeguarding the accuracy, integrity, confidentiality and security of the student information system and student’s academic records. It provides academic records services to students, faculty, staff, alumni and other government regulatory bodies in support of the mission and vision of the University.

The Registrar’s Office envisions itself as a leading support department in the delivery of quality service to all sectors of the academic community.
It shall continuously endeavor for the total development of its working force as highly skilled and professional individuals exhibiting a deep sense of dedication, loyalty, creativity, integrity and service.
It shall continuously work hand in hand with all the departments in the creation of a wholesome Lyceum family atmosphere and shall unceasingly support the institution in the pursuit of its mission.

Enrolment Procedure (New Students)
This starts with the interviewing/conducting of initial assessment of the applicants at the Academic Department and ends with the processing and issuance of ID.

Enrolment Procedure (Old Students)
This starts with the Advising of Subjects to the applicants at the Academic Department and ends with the payment of fees and issuance of registration form.
Credentials to Submit
- Valid School ID
- Grade Report
- Final permit of the previous semester attended
- Student Clearance (for Returning Students)
STEPS:
- Academic Department (Issuance of advising slip)
- Registrar’s Office (for IRREGULAR students) – Window’s 1 – 6, JPL Bldg. (Evaluation of Subjects advised by the Academic Department)
- Academic Department (Encoding of Subjects) – refer to STEP 1
- Cashier (Payment of Fees) – Ground Floor, JPL Bldg.
ISSUANCE OF DIPLOMA
Diploma is issued only once and is given during the Commencement Exercises; in case the diploma is lost or damaged, a second copy may be requested by the graduate at the Inactive Records Section of the Registrar’s Office.
Procedure on Requesting Second Copy of Diploma
- For lost diploma, secure an affidavit of loss and submit it to Window 4. For the damaged diploma, surrender the said document to Window 4.
- Fill out Request for Second Copy of Diploma.
- Pay the necessary amount to the Cashier.
- Submit the receipt to Window 4 and ask for the claim slip where in the schedule of release is indicated.
- Follow up after 3-4 weeks.
ISSUANCE OF CHED AUTHENTICATION (CAV/RED RIBBON)
Requirements
- For BS Nursing
- Original copy and one (1) clear/powder copy of Official Transcript of Records
- Original copy and one (1) clear/powder copy of Diploma
- Original copy and one (1) clear/powder copy of RLE
- For other courses
- Original copy and one (1) clear/powder copy of Official Transcript of Records
- Original copy and one (1) clear/powder copy of Diploma
For graduates before October 2009/Non-PACUCOA Accredited Programs
Additional to the above mentioned requirements:
original copy and one (1) clear/powder copy of Special Order (S.O.)
Procedure:
- Present the requirements at Window 4 for verification.
- Proceed to the Cashier for payment.
- Submit the receipt to Window 4
- For Marine Courses and or urgent request, the applicant is advisedto wait for the endorsement letter and authenticated documents to personally bring it to CHED.

Other option, our Liaison staff will bring the documents to CHED and advised the applicant to make a follow up after 2-3 weeks. The claim stub for the authentication can be claimed at Window 4 of Registrar’s Office after 2-3 weeks, but the authenticated document shall be claimed at the Department of Foreign Affairs (DFA).
ISSUANCE OF TRANSFER CREDENTIAL
Transfer credential or Honorable Dismissal shall be issued only once.
Procedure:
- Accomplish a student clearance and present it to the assigned window for a specific course.
- Pay to the Cashier the indicated amount.
- Submit the clearance together with the receipt to the window assigned to your course, a claim slip will be issued where in the schedule of release is indicated.
- Follow up after 2-3 weeks.
ISSUANCE OF OFFICIAL TRANSCRIPT OF RECORDS (TOR)
Newly graduate students can file for Official Transcript of Records (TOR) the day after the Commencement Exercises. Processing of TOR is 2-3 weeks from the date of filing.
Procedure:
- Accomplish a student clearance and present it to the assigned window for a specific course.
- Pay to the Cashier the indicated amount.
- Submit the clearance together with the receipt to the window assigned to your course, a claim slip will be issued where in the schedule of release is indicated.
- Follow up after 2-3 weeks.
VERIFICATION OF RECORDS
